Why do I need to fill in this form?

We require some information on you and your child in order to communicate with your referring doctor and to bill the appointment. You would usually fill in this information when you come to our rooms. To save you the trip we offer this online service as a courtesy to our telehealth clients.

Is this information safe to provide over the Internet?

Yes. We use the same data-encryption technology in use by online-shops and Internet banking. Your data is encrypted before it is sent to us and is safe from prying eyes.

What happens after I fill in the form?

Our office will be in contact with you to finalise details of the appointment. You will also receive a further email detailing the appointment and for you to give consent to bill Medicare.
